W12 Together, North H&F NHS Primary Care Network, and Hammersmith & Fulham Council are running a series of community focus groups to hear directly from local residents about their experiences of GP services.

Your views will help shape how services are developed, communicated, and delivered in the future, ensuring they meet the needs of our community.

Each session will:

• Last 2 hours

• Include refreshments

• Be facilitated in a relaxed, safe and respectful environment

We want to:

• Understand what is currently working well

• Identify barriers to access and communication

• Learn from lived experience to improve services

• Ensure North Hammersmith and Fulham primary care is shaped with local people
